The mediastinum harvests some of the most vital organs, therefore when fighting against infectious processes, knowledge of their relations with fascia and spaces of the neck, infra-abdominal viscera and superficial thoracic planes are of the outmost importance for the thoracic surgeon, clinician and radiologist; this has been a difficult subject to explain throughout the years since education about it has always been done in a superficial way during medical school and residency, the goal of this chapter is to present it in an easy to understand format, in a way that makes sense for the reader. A review of the anatomical configuration of the neck, mediastinum and the relation between them, clarifying the pathways from which infections can extend from above, below and superficially. Also, the anatomical landmarks that the surgeon needs to know in order to adequately locate the site of incision when an infection ensues. Finally it will analyze the theories on how microorganisms have the advantage when spreading from a site outside of the thoracic cavity, into it. A flow diagram is offered to explain all the points discussed in the chapter, from the sites of origin up until reaching the mediastinal spaces, correlating it with the pathways of spread for disease. © 2015 Nova Science Publishers, Inc.
